A Case of Akinetic Mutism due to a Bilateral Lenticulostriate Artery Infarction

Abstract

A 49-year-old man with a past history of diabetes mellitus and hypertension was admitted with left-sided weakness and dysarthria which had developed the day before. Diffusion-weighted MRI showed an acute infarction in the right basal ganglia and corona radiata. The symptom worsened on the next day, revealing right-sided weakness with akinetic mutism. Follow-up MRI showed a progressed infarction in the left basal ganglia and corona radiata, demonstrating the bilateral symmetrical lesions in the lenticulostriate artery territory. Akinetic mutism presented in this patient may result from a disruption of frontal-subcortical circuits. We report the patient with akinetic mutism and quadriparesis due to a bilateral lenticulostriate artery infarction.
